Twelve stitches by eight rows on a proper grid, with a 2/2 cable, two eyelets and a slipped stitch sitting in it, and a seven-symbol key underneath. Odd rows are numbered on the right, even rows on the left, because right-side rows are read right to left - the way the stitches sit on the needle. And the line at the bottom that every knitter already knows: the commonest symbol in the whole language is nothing at all. An empty square means knit on the right side and purl on the wrong side.
