The 5th Battalion, 46th Infantry, was originally constituted on 15 May 1917 in the Regular Army as Company E, 46th Infantry. October 1967 as Headquarters and Headquarters Company, 5th Battalion, 46th Infantry, and activated at Fort Hood, TX (its organic elements were concurrently constituted and activated). The battalion was assigned on 31 March 1968 to the 198th Infantry Brigade. It was relieved on 15 February 1969 from assignment to the 198th Infantry Brigade and assigned to the 23d Infantry Division for Vietnam Combat duty.

Sykes' Regulars have deployed three times in support of Operation Iraqi Freedom (OIF). 5th Battalion, 60th Infantry (Mechanized Dec. 1966 – 12 September 1968; Infantry 13 September 1968 – October 1970) Activated at Fort Riley, Kansas, and assigned to the 9th Infantry Division for its deployment to the Republic of Vietnam in December 1966. The 9th Division was the only major U.S. combat unit to conduct operations in the Mekong Delta. On 13 September 1968, the 5th/60th Mech was "swapped" with the 1st/16th of the 1st Infantry Division. From then until August 1970, the 5th/60th operated as a "straight leg" infantry unit.

From September through November 1966 the soldiers of "Tropic Lightning" participated in Operation Attleboro, East of Tay Ninh City, in War Zone C. In 1967, the 25th and 4th Divisions, both located in the Pleiku area swapped and interchanged men, equipment and commands of a number of units. By the end of 1970 a large portion of Tropic Lightning soldiers had returned stateside to their base at Schofield Barracks, Hawaii. In June 1966 during Operation Hawthorne, Company C, 2d Battalion, 502d Infantry was conducting a mission to locate elements of the North Vietnamese People's Army of Vietnam (PAVN) 24th Regiment. Charlie Company made contact with what was estimated to be a battalion-sized enemy element. Under heavy enemy fire and unable to maneuver in any direction, CPT Bill Carpenter called for air strikes on top of his position in an attempt to force the enemy to withdraw. "We might as well take some of them with us", he radioed to the 2nd Battalion command post. The Napalm attack injured seven of Carpenter's men, but the PAVN ceased fire long enough to allow Company C to consolidate, reorganize and establish a position

The Battle of FSB Mary Ann occurred when Viet Cong (VC) sappers attacked the U.S. firebase located in Quảng Tín Province, South Vietnam early on the morning of 28 March 1971. On the early morning of 28 March sappers from the VC 409th Sapper Battalion approached the wire of FSB Mary Ann and took up positions to launch an attack. The exact number of sappers involved is uncertain, but most sources agree that there were at least 50. As was common practice for such units, the sappers wore khaki shorts and soot camouflage and were armed with either an AK-47 or RPG-7 and satchel charges and grenades to attack bunkers. Sappers relied on stealth, shock and surprise to give them an advantage, and rarely carried heavy weapons or equipment.

During the Vietnam War, the 1st Battalion, 46th Infantry and the 5th Battalion, 46th Infantry were part of the Americal Division. Both initially deployed to Vietnam with the 198th Infantry Brigade from Fort Hood, Texas. But the 1-46th was placed under operational control of the 196th Infantry Brigade (Light) and later became fully a part of the 196th Brigade. Though the 5-46th left Vietnam when the Americal Division cased its colors in the fall of 1971, the 1-46th deployed to Da Nang with the rest of the 196th. The 1-46th left Vietnam in June 1972.
